발바닥근막염
발바닥근막염의 원인은 발바닥에 있는 족저근막이 손상을 입어 염증이 생기는 경우입니다.
족저근막은 발뒤꿈치 뼈에서 발가락까지 연결되어 있습니다.
대부분 발뒤꿈치 통증을 제일 많이 호소하지만 때로는 발 가운데 통증이나 앞발바닥 통증이 먼저 나타나기도 합니다.
발의 아치를 지지하고 충격을 흡수하는 족저근막이 과사용 되거나 큰 충격을 받으면 근막이 약간 찢어지는 미세손상이 생기면서 통증이 발생하게 됩니다. 찢어진 족저근막을 회복시켜주는 마땅한 치료법이 없기 때문에 환자들은 최소 몇 개월에서 몇 년씩 고생하는 경우가 많습니다.
앉아 있을 때는 발의 아치가 올라가 있고 근막이 느슨해진 상태를 유지하게 됩니다.
체중을 싣고 걸을 때는 아치가 내려오면서 족저근막이 늘어나게 됩니다.
앉아있거나 누워있을 때는 족저근막이 느슨한 상태를 유지하지만 일어서서 걸을 때는 아치가 내려오면서 족저근막이 늘어나게 됩니다.
찢어진 족저근막은 저녁에 잘때 느슨한 상태를 유지하면서 살짝 붙었다가 주로 아침에 첫발을 디딜 때 근막이 늘어나면서 다시 찢어지게 됩니다. 족저근막이 다시 찢어질때 통증을 가장 많이 느끼게 되며 조금 걷다 보면 상처가 벌어지면서 통증이 줄어듭니다.
앉아서 휴식을 취하면 근막이 느슨해지면서 다시 붙고 일어서면 또 찢어지면서 통증이 생깁니다.
한쪽 뒤꿈치가 아프면 반대쪽 뒤꿈치에 힘을 실어서 양쪽 뒤꿈치가 다 아파지고 양쪽이 다 아프면 발의 가운데나 앞쪽도 과사용을 하게 되어 발전체에 통증을 느끼는 분들도 많습니다.

갈비뼈(늑골)가 금이 가거나 부러질 경우 병원에서는 깁스도 해주지 않고 소염진통제만 처방합니다. 진통제로 통증을 참고 버티다 보면 우리 몸이 스스로 뼈를 붙이기 때문입니다.
찢어진 족저근막을 붙이는 치료법이 아직 없기 때문에 뒤꿈치 통증이 있는 분들은 소염진통제로 고통을 참으면서 우리 몸이 스스로 상처를 치유하기를 기다리는 수밖에 없습니다.

외국 족부 정형외과에서는 발사용을 줄이면 족저근막이 자연적으로 회복된다는 점에 착안하여 개개인의 발을 석고로 본떠서 통증 부위를 지면에 닿지 않도록 해주는 족부 맞춤깔창을 제작해서 사용합니다.
족부맞춤깔창은 발바닥 전체에 있는 족저근막을 고정시키고 뒤꿈치를 들어 올려서 통증 부위가 지면에 닿지 않고 족부 맞춤깔창 공간 안에 떠 있도록 설계합니다. 이렇게 설계된 족부맞춤깔창은 보행 시 통증 부위를 지면의 충격으로부터 보호해 줍니다.
족부맞춤깔창(Foot Orthotics)은 족부 정형외과가 있는 미국, 유럽 등 의료선진국에서는 의료보험으로 무상 제작 지원되는 제품입니다.
(국내에서는 아직 보험적용이 되지 않습니다. 외국인의 경우 자국 보험이 적용된다면 메디풋 제작 영수증으로 보험처리를 받으실 수 있습니다)
족저근막이 늘어나면 아직 아물지 못한 상처가 다시 찢어지게 되고 통증을 느끼게 됩니다.
보행할 때 아치가 내려오는 것을 막아줘야 근막이 늘어나면서 상처가 찢어지는것을 막을 수 있습니다.
자신의 발에 맞는 족부 맞춤깔창을 제작해서 착용하게 되면 발의 아치가 내려오는 것을 막아줘서 근막 늘어나지 못하도록 합니다. 마치 발에 반깁스한 것과 같은 효과를 나타냅니다.

발바닥 근막염 환자마다 통증 위치, 통증 정도가 다릅니다. 개인 증상에 따라 족부 맞춤깔창의 소재와 설계도 달라져야 효과적으로 발을 보호할 수 있습니다. 발바닥 근막염은 초기, 중기, 만성 환자로 구별되는데 만성 환자분들 중에는 발바닥의 지방층이 얇아져 있는 경우가 많습니다.
메디풋 족부 맞춤깔창은 발뒤꿈치의 지방층을 모아주는 역할을 함께 합니다. 본인의 지방층을 모아서 족저근막과 발뒤꿈치를 보호할 수 있습니다.

발뒤꿈치 충격 완화를 위해 특수 설계된 압력 조절 패드를 증상에 맞게 선택해서 제작됩니다. 압력 조절 패드는 증상과 통증 위치에 따라 다르게 설계되는데 통증 초기라면 3~4mm 압력조절패드를 사용하며 중기나 만성 환자분들은 최대 12mm까지 압력 조절 패드로 설계하여 발뒤꿈치를 보호할 수 있습니다.
-발뒤꿈치 부위 맞춤형 설계-
발바닥 근막염 환자 중에는 뒤꿈치뼈의 일부분이 석회화되어 가시처럼 뼈가 자라는 분이 있습니다. 뒤꿈치에 뼈가시(골극)가 생기면 아래에 있는 족저근막을 찔러 통증을 일으킵니다. 족부맞춤깔창은 뼈가시 아래에 도넛 모양의 패드를 넣어서 도넛 구멍 속으로 뼈가시가 들어가게 제작됩니다. 뼈가시가 공중에 떠 있는 듯한 효과를 내기 때문에 효과적으로 근막을 보호할 수 있습니다.

족부맞춤깔창은 평소 착용하는 운동화, 구두, 슬리퍼, 작업화 등 대부분의 신발에 착용 가능합니다. 활동하는 모든 시간동안 착용해야 발이 쉬는 듯한 효과가 유지되고 발을 보호합니다.
운동화 안에 있는 기존깔창을 제거한 뒤 족부맞춤깔창을 착용하면 됩니다.
기존깔창이 없거나 제거 되지 않는 신발(구두,단화,스니커즈,컨버스화 등)은 구두 및 슬리퍼용 제품을 넣어 착용하면 됩니다.
슬리퍼나 샌들에 고정할 수 있습니다. 보행 시 족부맞춤깔창이 슬리퍼에서 떨어지지 않습니다.
족부맞춤깔창은 평소 착용하는 운동화, 구두, 슬리퍼, 작업화 등 대부분의 신발에 착용 가능합니다.
활동하는 모든 시간동안 착용해야 발이 쉬는 듯한 효과가 유지되고 발을 보호합니다.
족부 맞춤깔창은 제작할 때 아주 정교함이 요구되는 작업이기 때문에 반드시 본인이 직접 본사로 방문하셔서 석고로 발본을 뜬 후 제작해야 합니다.
사람마다 발의 아치 굴곡이 다르고 나이, 발의 형태, 통증 위치가 다르기 때문에 개개인의 특성에 맞게 맞춤깔창의 소재나 두께가 다르게 설계됩니다.
메디풋에서는 족부라는 말도 생소하던 지난 2002년부터 족부전문병원, 대학병원 납품을 시작으로 수많은 환자분들과 국가대표, 프로 운동선수들의 족부 맞춤깔창을 제작해오고 있습니다.
최근 제품들은 환자의 증상이나 나이, 체중 등을 고려하여 제품의 탄성을 12단계로 조절할 수 있으며 질환별로 다양한 쿠션과 패드를 선택할 수 있습니다.
